The world of youth sports is as thrilling as it is challenging, and with it comes the reality of overuse injuries. These injuries occur when repetitive forces from a specific sport exceed the body’s ability to repair itself. Unlike adult athletes, children haven’t fully matured physically, which means they can be particularly vulnerable to such stresses.
Causes and Contributors
Overuse injuries often arise from rapid increases in training duration or intensity, sudden changes in training surfaces, or using worn-out gear. Additionally, intrinsic or biomechanical factors such as lower extremity rotational misalignment, genu varum or valgum, excessive subtalar joint pronation, and residual metatarsus adductus can contribute to these injuries. Conducting a thorough biomechanical examination can help not only treat the injury but also address its root cause.
Understanding Stress Fractures
Stress fractures occur when repetitive stress surpasses the bone’s capacity to heal itself. Although the tibia is the most common site for stress fractures in both children and adults, the distal fibula is more frequently affected in children. Children’s bones generally heal faster than adults, necessitating frequent monitoring. Stress fractures manifest as gradual pain that intensifies with activity and eases with rest.
Diagnosing stress fractures typically involves X-rays, while MRIs can provide earlier detection. Differentiating stress fractures from medial tibial stress syndrome (MTSS) is crucial, as their treatments differ. High-risk stress fractures, such as those in the tibia’s anterior cortex, require aggressive treatment to prevent complications. Returning to sports should only occur once healing is confirmed through clinical and radiologic evidence.
